Veneer revision — the European standard
The European-standard clinical pathway for revising failing, aged, or aesthetically-disappointing veneer and crown cases at ACE DNTL STUDIO Marbella, authored by Dr. Ace Korkchi, DDS (University of Gothenburg, 2009). Six clinical signs indicate revision is worth considering: marginal staining at the gum margin (visible dark line); persistent sensitivity at 12+ months post-delivery; debonded restorations; chipping or fracture of the porcelain; gum recession exposing the prep margin; aesthetic dissatisfaction. The revision pathway runs in four stages. Stage 1 — Diagnostic and assessment: clinical examination under loupe magnification, intraoral photography in three lighting conditions, periodontal probing, 3D CBCT imaging where indicated, Aesthete Test scored across its 12 dimensions to determine candidacy. Stage 2 — Foundational work where indicated: endodontic treatment for pulp pathology, periodontal management for recession, occlusal stabilisation for parafunction. Stage 3 — Replacement: existing restorations removed under loupe magnification with minimal further preparation, new restorations planned via Digital Smile Design and mocked-up per the ACE Mock-Up Protocol (14-day try-in), final porcelain hand-layered Signature from ACE DNTL LAB or pressed e.max per the Material Hierarchy. Stage 4 — Structured follow-up at 6 months / 2 years / 5 years against the ACE Smile Index™ (DOI 10.5281/zenodo.19634136). Specific revision pattern documented for Turkey-teeth cases (aggressive preparation, monolithic crowns, compressed timeline — see /turkey-teeth-explained) and for cases requiring extraction-and-implant on severely-compromised tooth structure. International-patient revision logistics: Costa del Sol 2–3 visit protocol, remote pre-consultation before any travel, pre-arrival imaging requirements. Some cases are accepted directly; some require staged work; some are refused under the Refusal Doctrine where further aggressive preparation on already-compromised structure cannot be safely performed.

Direct Answers
- Can old or failing veneers be fixed?
- Yes, in most cases. The pathway depends on what is underneath the current restorations and on the periodontal foundation. Cases where the pulp is healthy and gum tissue stable can typically be re-treated. Cases requiring foundational work proceed with endodontics or periodontal therapy first. Each case is assessed individually against the Aesthete Test.
- How do I know if my veneers need revision?
- Six signs: marginal staining at gum line, persistent sensitivity past 12 months, debonded veneers, porcelain chipping/fracture, gum recession exposing the prep margin, aesthetic dissatisfaction. Any one warrants a clinical re-score; multiple signs warrant a full revision-pathway consultation.
- Is revision more expensive than the original treatment?
- Sometimes — but not always. Replacement of existing crowns runs €600–€900 per veneer (same as fresh case). Endodontic treatment adds €400–€800 per affected tooth. Periodontal management adds €1,000–€3,000. Full quote given after diagnostic consultation.
- Can Turkey teeth be reversed back to natural teeth?
- Only sometimes — and rarely fully. Aggressive preparation removed structure that cannot be replaced. Realistic options: replace with European-standard hand-layered crowns; extract-and-implant for severely compromised teeth; leave in place with structured follow-up if clinically acceptable. Returning to pre-treatment natural teeth is almost never possible after aggressive prep.
- Will ACE DNTL refuse my revision case?
- Some cases are refused under the Refusal Doctrine — cases requiring further aggressive preparation on already-compromised structure, cases where the periodontal foundation cannot be stabilised, cases where patient expectation does not align with biological reality. Most cases are accepted. The diagnostic consultation determines the category.
- How long does revision take?
- Same 2–3 visit Costa del Sol protocol as new cases (~30 days), extended by foundational work where required (60–120 days). International patients accommodated with remote pre-consultation, fitness-for-travel assessment, and the mock-up phase between visits.
